You don't have to have a Masters to get a PhD.

You will need to find the school that offers a PhD in your field of study. You will need to take the GRE general test and possibly a specific test depending on the school and major. The best way to find out what school offers PhDs in your major?

If you are an university student, your department should have information about what graduate schools that offer PhDs are available. Ask a professor that teaches in your major and ask your adviser. It is not difficult to find out.

Yes, you have to go to schools which offer Ph.D. programs. You can find out on the web my googling Ph.D. program history, for example, or you can look at the websites of the schools around you to know which ones offer the Ph.D. in your field. More usefully, because they can tell you which ones are good, you can ask professors in your undergraduate or master's program about which schools offer good Ph.D. programs in your field (they will almost invariably recommend their own school among them, though, and this one isn't always that good!).Usually those offering Ph.D. programs are the flagship research institutions within the state system (for example, in California, the UCs offer Ph.D.s, the CSUs, so far, do not), or the larger, more prestigious private institutions. Not every subject matter is taught at every school which offers a Ph.D.

You have to have your bachelors and masters.

The whole process to get into a PhD program at a university is quite difficult. You need reccomendations, essays, etc. etc. etc.

They have PhD programs at various universities. They have the CUNY graduate center in new york, NYU, Columbia, Harvard, Yale, etc. etc.
