How to phrase email to ask professor to meet for office hours?

My professor has office hours by appointment, otherwise I would just go when he said. I'm emailing him to ask when he would be available, but it all sounds really awkward to me (writing is not my string suit). Basically, I just want to ask if/when he would be available to meet. Suggestions on how to phrase that, casually but still respectfully?

I would like to meet with you because I have some questions about Chapter 16. I will be free Monday after 3 pm, Tuesday between 11 am and 2 pm, and Wednesday before noon. Would any of those times work for you?
